CS10 Blend (Cagrilintide + Semaglutide)?

CS10 is a ready-made combination of two receptor agonists: the amylin analog Cagrilintide (5 mg) and the GLP-1 receptor agonist Semaglutide (5 mg) in a single vial. In the clinical literature the cagrilintide + semaglutide combination is studied in the context of obesity and metabolic disorders.

CS10 combines two separate receptor systems. Semaglutide acts on the incretin axis (the GLP-1 receptor), and cagrilintide — on the amylin pathway. Because these are different receptor mechanisms, the combination is studied as simultaneous signaling at both receptor systems. This distinguishes CS10 from single agonists, such as Tirzepatide or Retatrutide, which combine incretin axes within a single molecule.

CS10 (cagrilintide + semaglutide) is studied in the context of obesity (the REDEFINE 1 program) and type 2 diabetes (REDEFINE 2). According to publicly available data it remains an experimental molecule and holds no registration. In the EU it is not authorized for clinical use. See also the other peptides for weight loss (GLP-1) in our category.

In the version offered by Peptology, CS10 is supplied as a lyophilized research peptide with purity above 99% (HPLC/MS), confirmed by independent laboratory analysis (CoA for every batch), in a fixed composition of Cagrilintide 5 mg + Semaglutide 5 mg. The product is intended exclusively for scientific research and in vitro applications and is not a medicinal product.

Composition: Cagrilintide 5 mg + Semaglutide 5 mg

CS10 is a fixed combination of two peptides in a single lyophilized vial — 10 mg of peptide substance in total. Each component acts on a different pathway, and together they form a dual amylin + GLP-1 mechanism.

CAGRILINTIDE 5 MG

Amylin analog

Activates the amylin receptors (AMY1, AMY3) and the calcitonin receptor. See the single product.

SEMAGLUTIDE 5 MG

GLP-1 agonist

Activates the GLP-1 receptor; studied for glucose-dependent insulin secretion and gastric emptying. See the single product.

The fixed 5 mg + 5 mg composition has purity > 99% and a laboratory certificate for the batch. Both components are reconstituted simultaneously in one vial.

How CS10 works — the dual amylin + GLP-1 pathway

In clinical publications, the cagrilintide + semaglutide combination is described as simultaneous activation of two independent receptor systems — the amylin and calcitonin receptors and the GLP-1 receptor. Because amylin and GLP-1 stimulate different receptors, the two pathways complement each other.

GLP-1 AXIS (SEMAGLUTIDE)

Activates the GLP-1 receptor, including in central pathways; linked in the literature to slowed gastric emptying and glucose-dependent insulin secretion.

AMYLIN AXIS (CAGRILINTIDE)

Activates the amylin receptors (AMY1, AMY3) and the calcitonin receptor — on a pathway separate from the incretins.

ADDITIVE EFFECT

In the REDEFINE 1 study the combination produced ~20.4% weight reduction — more than cagrilintide alone (11.5%) or semaglutide alone (14.9%).

Why combining two pathways matters

Single molecules — even multi-receptor incretins like Tirzepatide — act within the incretin system. CS10 adds a completely different pathway (amylin), which studies examine as simultaneous signaling at two independent receptor systems. This makes CS10 a model research tool for studying combined amylin + GLP-1 strategies.

This concept is being studied in an extensive phase 3 program (REDEFINE 1 in obesity, REDEFINE 2 in type 2 diabetes). The results presented come from controlled clinical trials and do not constitute a claim about the effect of the research-grade product.

Pharmacokinetics and pharmacodynamics of CS10

Both components of CS10 are acylated peptides with a long half-life thanks to albumin binding — which explains the convenient, weekly dosing schedule used in clinical protocols for the combination.

  • Half-life (semaglutide): about 7 days (~168 h)
  • Half-life (cagrilintide): about 7–8 days (~180 h)
  • Dosing schedule: once a week subcutaneously (SC), with dose titration over time
  • Long-action mechanism: binding of both components to albumin via fatty-acid residues
  • Profile: matched half-lives allow the combination to be administered in a single, weekly schedule
  • Doses studied in phase 3 (combination): cagrilintide 2.4 mg + semaglutide 2.4 mg SC weekly, with titration

CS10 vs Semaglutide vs Cagrilintide

The table below compares the CS10 combination with its two single components. This is the most telling comparison, because it comes from a single study (REDEFINE 1), in which all three arms were assessed in parallel — and it shows the additive effect of two pathways. The data are scientific and informational in nature.

Sources: the REDEFINE 1 program (obesity, n≈3417) and REDEFINE 2 (type 2 diabetes, n≈1206) for the cagrilintide + semaglutide combination (NEJM 2025). All arms of REDEFINE 1 were assessed in a single study, which makes this comparison consistent; it does not, however, constitute a claim about the effect of the research-grade product.

Clinical trials of CS10 (cagrilintide + semaglutide)

The clinical program of the cagrilintide + semaglutide combination is being developed in two main phase 3 studies — REDEFINE 1 (obesity) and REDEFINE 2 (type 2 diabetes). These results come from controlled clinical trials.

Scientific basis — the single components

Each component of CS10 has its own clinical program: semaglutide in the STEP (obesity) and SUSTAIN (type 2 diabetes) studies, and cagrilintide in a dose-finding phase 2 study in obesity. The combination was created to combine two independent receptor systems in a single, weekly schedule.

REDEFINE 1 — obesity (NEJM, 2025)

In the phase 3 study REDEFINE 1 (about 3417 adults with obesity without diabetes, 68 weeks), the cagrilintide + semaglutide combination produced a mean weight reduction of 20.4%, versus 11.5% for cagrilintide alone, 14.9% for semaglutide alone and 3.0% for placebo. A weight reduction of ≥ 20% was achieved by about 60% of participants, and ≥ 30% — by about 23%. These results come from a controlled clinical trial and do not constitute a recommendation for individual use.

REDEFINE 2 — type 2 diabetes (NEJM, 2025)

In the REDEFINE 2 study (about 1206 adults with type 2 diabetes and overweight/obesity, 68 weeks), the combination produced a weight reduction of −15.7% versus −3.1% in the placebo group, with a significant improvement in glycemic control. A weight reduction of ≥ 15% was achieved by 51.6% of participants. These results come from a controlled clinical trial.

Note: The information on clinical trials concerns the cagrilintide + semaglutide combination and is presented for scientific and informational purposes. It does not constitute medical advice, a recommendation for use or a claim about the effect of the research-grade product offered by Peptology.

Research dosing protocols

Important: The information below describes doses used in published clinical trials of the cagrilintide + semaglutide combination. It is provided for scientific and informational purposes and does not constitute a recommendation for individual use, medical advice or administration instructions. The research-grade CS10 offered by Peptology is not a registered medicine.

In the REDEFINE clinical program, the combination was used in a weekly subcutaneous schedule with gradual dose titration — which aims to improve gastrointestinal tolerability:

  • Components: cagrilintide + semaglutide administered simultaneously once a week SC
  • Phase 3 (REDEFINE): target dose cagrilintide 2.4 mg + semaglutide 2.4 mg SC weekly
  • Titration schedule: a slow dose increase over several weeks to improve tolerability
  • Reconstitution: both components in one vial — they are reconstituted together with a single solvent

Like the single molecules — Semaglutide and Cagrilintide — CS10 in studies requires slow titration to improve tolerability. These schedules are part of controlled clinical trials and do not constitute a recommendation for self-administration.

Storage and reconstitution

CS10 is supplied as a lyophilized (freeze-dried) powder in a vial containing both components. In lyophilized form the peptides are stable at room temperature, protected from light and moisture — refrigeration before reconstitution is not mandatory. After reconstitution, stability depends on the solvent used, with Peptide Matrix Water™ significantly extending the peptides' lifespan.

  • Before reconstitution: room temperature, in the original packaging, protected from light and moisture. A refrigerator is not mandatory.
  • After reconstitution with Peptide Matrix Water™: stability in the refrigerator +2 °C to +8 °C for up to 60 days under appropriate laboratory conditions — the stabilizing formula extends the peptides' lifespan relative to standard solvents.
  • After reconstitution with bacteriostatic or sterile water: stability in the refrigerator +2 °C to +8 °C for up to 28 days under appropriate conditions. Do not refreeze the reconstituted solution.
  • Reconstitution: both components are reconstituted simultaneously in one vial; Peptide Matrix Water™, bacteriostatic water or sterile water for injection is usually used.
  • Volume: maximum 3 mL of solvent (capacity of a standard vial). For a precise calculation of volume and units, use the peptide calculator.
  • Homogenization: add the solvent slowly down the vial wall, without vigorous shaking — peptide bonds are sensitive to mechanical stress. Swirl gently, do not shake.

Laboratory tip: With your order, you can add Peptide Matrix Water™ to the cart — a stabilizing solution designed for working with reconstituted research peptides, including GLP-1 / amylin combinations. More in the article Bacteriostatic water vs acetic acid vs matrix water.

Parameter Value
Form Lyophilized powder
Solvent Peptide Matrix Water
Reconstitution volume 1–3 mL — maximum 3 mL (capacity of the standard vial)
Storage before reconstitution Room temperature, protected from direct light (refrigeration not required)

What you need

Precision pipette

Calibrated, 100–1000 µL — for accurately measuring the volume of solvent added to the vial.

70% alcohol swabs

Two swabs — to disinfect the peptide vial and the solvent vial.

Peptide Matrix Water

Stabilized solvent for lyophilized peptides. Why this one.

Five rules for reconstitution

✅ Correct ❌ Mistake
Disinfect the vial caps and the work surface with 70% alcohol; wear gloves Working without disinfection
Add the solvent slowly along the vial wall Direct jet onto the powder — foaming and denaturation
Gentle rolling between the palms Shaking — foam, which denatures peptides
10–15 minutes waiting at room temperature Mechanical mixing or heating
Visual check: clear, particle-free Working with a cloudy or discolored solution

Calculating concentration

⚠️ Maximum 3 mL solvent. The standard vial does not accommodate larger volumes — do not exceed during reconstitution.

  • Concentration (mg/mL) = mg in the vial ÷ mL of solvent
  • Volume per sample (mL) = amount per sample (mg) ÷ concentration (mg/mL)
  • 1 mg/mL = 1 µg/µL · 1 mL = 1000 µL

👉 For any vial and solvent volume — use the calculator. It calculates the concentration (mg/mL, µg/µL) and the volume per sample (mL, µL).

Storage

BEFORE RECONSTITUTION

Room temperature, protected from direct light, in original packaging. Refrigerator not required.

AFTER RECONSTITUTION

+2 °C to +8 °C, up to 60 days with Peptide Matrix Water. Discard any remainder after this period.

FREEZING

Not recommended. Freeze/thaw cycles degrade peptide structure.

Four mistakes to avoid

1. Shaking "to dissolve faster"

Foam = denaturation. Only gentle rolling between the palms.

2. Inappropriate solvent

For peptide stability use only sterile, specially formulated solvent — recommended Peptide Matrix Water.

3. Ignoring slight turbidity

Not fully clear = the sample does not meet the criteria for use.

4. Taking it out of the refrigerator "for an hour–two"

Short exposures add up and compromise stability.
