What is Tesamorelin?

Tesamorelin is a synthetic, stabilized GHRH analog — the growth-hormone-releasing hormone. Structurally it is GHRH (1-44) with a trans-3-hexenoic acid modification at the N-terminus, which protects the molecule from enzymatic breakdown (including by DPP-4) and extends its action relative to native GHRH. It binds the GHRH receptor in the pituitary with high affinity and stimulates endogenous secretion of growth hormone (GH), and indirectly IGF-1.

The feature that distinguishes Tesamorelin among GHRH analogs is its selective action on visceral fat. A longer half-life (~26 min, versus ~12 min for Sermorelin) provides more sustained GHRH signaling, which in clinical trials translated into a marked reduction of visceral adipose tissue (VAT). This sets it apart from GHRH analogs directed mainly at the general GH axis, like CJC-1295.

The research-grade Tesamorelin offered by Peptology is not a medicinal product — it is a lyophilized research peptide intended exclusively for scientific and in vitro applications, not intended for use in humans or animals. In the EU it is not authorized for clinical use. See also the other recovery and anabolic peptides in our category.

In the version offered by Peptology, Tesamorelin is supplied as a lyophilized peptide with purity above 99% (HPLC/MS), confirmed by independent laboratory analysis (CoA for every batch). It is available in two vial formats — 10 mg and 30 mg — depending on the scale of the research protocol.

Available formats: 10 mg and 30 mg

Tesamorelin is available in two lyophilized formats that cover different scales of research work — from single in vitro series to longer GH-axis and lipolysis protocols with higher peptide consumption.

10 MG

Standard format

Suitable for validation experiments, single in vitro series and shorter GH-axis protocols.

30 MG

Enlarged format

For longer research series and lipolysis/body-composition protocols with higher peptide consumption — justified by a daily schedule.

⚠ Important for Tesamorelin 30 mg: Due to its high concentration, this is a special, highly concentrated peptide. It is entirely normal for the solution to look slightly milky/whitish after reconstitution. It is NOT normal, however, for the solution to be yellow or brown or to contain visible particles — in that case it should not be used.

Both formats have identical purity > 99% and the same laboratory certificate for the batch. The choice of size depends on the scale of the protocol, not on the concentration of the active substance.

How Tesamorelin works — stabilized stimulation of the GHRH axis

In the clinical literature, Tesamorelin is described as a stabilized GHRH analog with a particular affinity for reducing visceral fat. The three features below together explain its profile — physiological, but distinctly lipolytic.

GHRH RECEPTOR

Binds the GHRH receptor in the pituitary with high affinity, stimulating endogenous GH secretion and indirectly raising IGF-1.

STABILIZATION

The trans-3-hexenoic acid modification confers resistance to enzymatic breakdown — a half-life of ~26 min, longer than native GHRH.

VISCERAL LIPOLYSIS

Sustained GH signaling translates in studies into a selective reduction of visceral adipose tissue (VAT) and liver fat.

Why selectivity for visceral fat matters

Visceral fat (VAT) — surrounding the abdominal organs — is metabolically active and strongly linked to cardiometabolic risk. Thanks to sustained, physiological stimulation of the GH axis, Tesamorelin selectively reduces VAT and liver fat in studies, with only a moderate effect on lean mass. This makes it a model research tool for studying GH-dependent lipolysis and the regulation of body composition.

Like other GHRH analogs, in studies Tesamorelin is sometimes combined with a GHRP secretagogue (e.g. Ipamorelin) acting on a separate receptor, GHS-R1a. These data come from controlled clinical trials and do not constitute a claim about the effect of the research-grade product.

Pharmacokinetics and pharmacodynamics of Tesamorelin

Tesamorelin is a stabilized GHRH analog which, in the published pharmacokinetic data, is described as a molecule with a short but slightly longer-than-native-GHRH half-life — which provides sustained signaling and justifies a daily dosing schedule.

  • Biological half-life: about 26 minutes (versus ~12 min for sermorelin)
  • Stabilization mechanism: trans-3-hexenoic acid modification (N-terminus) → resistance to DPP-4 and proteases
  • Profile of action: sustained GHRH signaling, physiological GH secretion
  • Target: GHRH receptor in the pituitary (cAMP pathway)
  • Research schedule: clinical protocols used 2 mg SC once daily
  • Route of administration: subcutaneous (SC), per the research protocol

Clinical trials of Tesamorelin

Unlike many research peptides, Tesamorelin went through a full clinical program. Below are the most important observations from the studies. They concern the pharmaceutical form and come from controlled clinical trials.

Phase 3 — HIV-associated lipodystrophy

In randomized, controlled phase 3 trials in patients with excess visceral adipose tissue in the course of HIV-associated lipodystrophy, daily administration of 2 mg of tesamorelin SC produced a significant reduction of visceral adipose tissue (VAT) — on the order of ~15–18% — along with a reduction in waist circumference and liver fat, with a moderate increase in lean mass and an improvement in the triglyceride profile.

Liver fat (NAFLD / NASH)

In subsequent studies, Tesamorelin was evaluated for the reduction of liver fat content in people with HIV and hepatic steatosis, with promising results regarding a decrease in the liver fat fraction. These directions remain the subject of further research.

Safety profile and metabolism

In meta-analyses of randomized trials, the selective effect on central fat was achieved without significant worsening of lipid parameters and — in most analyses — without a meaningful increase in risk. Because of the GH-dependent mechanism, glucose metabolism remains an important element of observation (a possible rise in blood glucose).

Research dosing protocols

Important: The information below describes doses used in published clinical trials of tesamorelin. It is provided for scientific and informational purposes and does not constitute a recommendation for individual use, medical advice or administration instructions. The research-grade Tesamorelin offered by Peptology is not a registered medicine.

In the clinical program, Tesamorelin was used in a simple, daily subcutaneous schedule — without complex titration:

  • Phase 3 (HIV lipodystrophy): 2 mg SC once daily as the studied therapeutic dose
  • Schedule: daily administration, usually in the evening, in a simple protocol
  • Alternative schedules were described (e.g. 5 days a week), but with potentially lower efficacy
  • Route of administration: subcutaneous (SC), per the research protocol

Like other GHRH analogs — CJC-1295 or Sermorelin — Tesamorelin is sometimes combined in studies with a GHRP secretagogue, like Ipamorelin. These schedules are part of controlled clinical trials and do not constitute a recommendation for self-administration.

Side effects and safety

In clinical trials, Tesamorelin was generally well tolerated, with predominantly mild to moderate side effects. Because of the GH-dependent mechanism, some effects are related to the GH/IGF-1 axis (including edema, joint pain, blood glucose).

Frequently observed in studies

  • Injection-site reactions
  • Joint pain (arthralgia) or muscle pain
  • Peripheral edema (water retention)
  • Headache

Less frequently described

  • A sensation of „stiffness" / tightness
  • Paresthesia (tingling in the limbs)
  • Nausea
  • Fatigue

Requires attention

  • Elevated blood glucose / insulin resistance (GH axis)
  • Carpal tunnel syndrome symptoms
  • Hypersensitivity to formula components

Most side effects in the studies were transient and resolved after dose adjustment. Because of the effect on glucose, blood glucose control is an important element of observation. This information is scientific in nature and does not constitute an instruction for use.

Storage and reconstitution

Tesamorelin is supplied as a lyophilized (freeze-dried) powder in a vial. In lyophilized form the peptide is stable at room temperature, protected from light and moisture — refrigeration before reconstitution is not mandatory. After reconstitution, stability depends on the solvent used, with Peptide Matrix Water™ significantly extending the peptide's lifespan.

  • Before reconstitution: room temperature, in the original packaging, protected from light and moisture. A refrigerator is not mandatory.
  • After reconstitution with Peptide Matrix Water™: stability in the refrigerator +2 °C to +8 °C for up to 60 days under appropriate laboratory conditions — the stabilizing formula extends the peptide's lifespan relative to standard solvents.
  • After reconstitution with bacteriostatic or sterile water: stability in the refrigerator +2 °C to +8 °C for up to 28 days under appropriate conditions. Do not refreeze the reconstituted solution.
  • Reconstitution: in a laboratory setting, Peptide Matrix Water™, bacteriostatic water or sterile water for injection is usually used, depending on the protocol.
  • Volume: maximum 3 mL of solvent (capacity of a standard vial). For a precise calculation of volume and units, use the peptide calculator.
  • Homogenization: add the solvent slowly down the vial wall, without vigorous shaking — peptide bonds are sensitive to mechanical stress. Swirl gently, do not shake.

Laboratory tip: With your order, you can add Peptide Matrix Water™ to the cart — a stabilizing solution designed for working with reconstituted research peptides. More in the article Bacteriostatic water vs acetic acid vs matrix water.

What you need

Precision pipette

Calibrated, 100–1000 µL — for accurately measuring the volume of solvent added to the vial.

70% alcohol swabs

Two swabs — to disinfect the peptide vial and the solvent vial.

Peptide Matrix Water

Stabilized solvent for lyophilized peptides. Why this one.

Five rules for reconstitution

✅ Correct ❌ Mistake
Disinfect the vial caps and the work surface with 70% alcohol; wear gloves Working without disinfection
Add the solvent slowly along the vial wall Direct jet onto the powder — foaming and denaturation
Gentle rolling between the palms Shaking — foam, which denatures peptides
10–15 minutes waiting at room temperature Mechanical mixing or heating
Visual check: clear, particle-free Working with a cloudy or discolored solution

Calculating concentration

⚠️ Maximum 3 mL solvent. The standard vial does not accommodate larger volumes — do not exceed during reconstitution.

  • Concentration (mg/mL) = mg in the vial ÷ mL of solvent
  • Volume per sample (mL) = amount per sample (mg) ÷ concentration (mg/mL)
  • 1 mg/mL = 1 µg/µL · 1 mL = 1000 µL

Reconstitution per vial — solvent: Peptide Matrix Water. Concentration after adding the solvent (1 mg/mL = 1 µg/µL):

Vial+ 1 mL+ 2 mL+ 3 mL
10 mg10 mg/mL5 mg/mL3.33 mg/mL
30 mg30 mg/mL15 mg/mL10 mg/mL

👉 For any vial and solvent volume — use the calculator. It calculates the concentration (mg/mL, µg/µL) and the volume per sample (mL, µL).

Storage

BEFORE RECONSTITUTION

Room temperature, protected from direct light, in original packaging. Refrigerator not required.

AFTER RECONSTITUTION

+2 °C to +8 °C, up to 60 days with Peptide Matrix Water. Discard any remainder after this period.

FREEZING

Not recommended. Freeze/thaw cycles degrade peptide structure.

Four mistakes to avoid

1. Shaking "to dissolve faster"

Foam = denaturation. Only gentle rolling between the palms.

2. Inappropriate solvent

For peptide stability use only sterile, specially formulated solvent — recommended Peptide Matrix Water.

3. Ignoring slight turbidity

Not fully clear = the sample does not meet the criteria for use.

4. Taking it out of the refrigerator "for an hour–two"

Short exposures add up and compromise stability.
